Go to Library > Forms. The table lists Form Name, Slug, Status (Active or Inactive), Submissions count, Created, plus per-row icons for Edit form, View submissions and delete. Click "Add New Form" in the header to build one. The Forms module only appears when form building is enabled for your account; viewing needs view_form.
