Open /app in the phone browser and use the install sheet. On iPhone: open the page in Safari (Chrome on iPhone cannot install apps), tap the Share icon, scroll to "Add to Home Screen", tap Add. On Android: tap "Install app" or "Add to Home screen" and confirm. You can also reach the instructions later from Settings > Install inside the app.
